Found scattered on ground in oak-pine forests.
Cap: Up to 6"; white with pyramidal warts over entire cap; convex becoming nearly flat; margin may have veil fragments; little or no odor. Gills: White/yellow-tinged, crowded. Stalk: White; fragile skirt-like ring; concentric down-curved scales near base; large spindle-shaped rooting bulb (J. Solem, pers. comm.).
